Completing the HEM Requirements

Employees who enroll in the HEM and their  spouse/domestic partner who also are enrolled in the HEM must complete the Health Engagement Model (HEMi) requirements to avoid being assessed the HEM surcharge by the applicable deadlines.

CHANGES effective May 2012:

  • New opportunities to enroll for employees that did not enroll during Open Enrollment, see Special Enrollment Period(s) 
  • Adds new enrollment tiers – allows for one member of a couple to participate in the HEM and the other member not to participate (surcharge will be based on an individual basis).

HEM Deadlines:

New Employees or Newly Eligible for Benefits in 2012:

  • Health Assessment Survey must be completed within 45 day after the start of the plan coverage.
  • Two online e-lessons must be completed within 195 days of the start of plan coverage.

Special Enrollment Period(s) for Employees that did NOT enroll in the HEM during Open Enrollment:

  • May 7th through May 18, 2012 – complete the health assessment by May 31st and your HEM Surcharge will end with your July coverage (last charge will be on your May paycheck).
  • July 1st or after – HEM surcharge will end with your participation start date depending on the date the form is received.
    • Received by the 15th of the month – surcharge will end that month
    • Received on or after the 16th of the month – surcharge will end the following month
    • The Health Assessment Survey must be completed within 45 days of your start date

  • Employees enrolled in the HEM during Open Enrollment – HEM start date January 1, 2012:Health Assessment Survey deadline  was March 31, 2012. If you or your enrolled spouse/domestic partner did NOT meet this deadline, the surcharge will begin on your June 30th paycheck.
  • Two online e-lessons must be completed by July 15, 2012.

HEM Surcharges – Not Enrolled or Missed Deadlines:

  • Effective July 1, 2012, surcharge amounts will be:
    • $17.50 per enrolled individual (employee or spouse/domestic partner) that is not participating or missed the deadline (maximum $35 per month)
    • Part-time Classified employees only:  $10.00 per enrolled individual (employee or spouse/domestic partner) that is not participating or missed the deadline (maximum $20 per month)

Note:  PEBB has changed to a per individual amount based on the time of non-enrollment or missed deadline rather than a yearly surcharge amount to be spread over the number of remaining months in the calendar year.

Steps to complete HEM requirements:

  1. Register with your insurance provider, you will need to have your medical insurance card available
  2. Complete the Health Survey
    • Referred to as:
      • “Your Personal Health Assessment” by Providence
      • “Succeed” or “Total Health Assessment” by Kaiser
    • Minimum requirements are to answer the following questions:
      • Gender
      • Age
      • Waist Circumference
    • NEW! To participate in Weight Watchers, only the minimum survey questions are required to be completed (gender, age, waist circumference
    • The survey can be saved for later completion and/or revision
    • Once the survey is submitted, the survey cannot be accessed again
  3. Complete two online e-lessons.
    • Providence:  Providence refers to the E-lessons as “Health Conversations” and are found under the “My Health - Self-Help” quick link (along the left hand side of the page).  E-lessons are narrated and available as closed captioned and in a format for the visually impaired.

      After registering and logging into the myProvidence site, you should see “Welcome, PEBB Member!”.  If you only see “Welcome!” follow these  steps:
    •  
      • Click on “My Preferences - Services” quick link (along the left hand side of the page)
      • Under “Add a New Service”,  click on “Sign up for health plan (member) services”
      • Enter your Member ID and Group Number from your Providence medical card (PEBB Statewide or Providence Choice)
      • Click on “Submit”
      • You will now need to “Log Out” (found at the top of the page)
      • Log back into myProvidence (you should now see “Welcome, PEBB Member!" and have access to the health assessment and e-lessons)
    • *Providence launched a new myProvidence website on December 1, 2011. If you established an account with myProvidence prior to December 1, 2011 and you don't remember your user name or password you need to call Providence at 877.569.7768 or 503.216.6463 and request your user name and a temporary password before you log into the website.

      If you have technical issues with the myProvidence website, please call 877.569.7768 or 503.216.6463.  For example, the links for your "Personal Health Assessment" and "Self-Help" (e-lessons) do not appear in the "My Health" section when you log in.
    • Kaiser:  E-lessons are video and other activities.  Kaiser refers to these as “Coaching Sessions” and “Healthy Lifestyle Programs”
